We expect all of our patients to be responsible for keeping their accounts with us current.  If you have a question about your bill, call 888-340-3330 and leave a message.  A billing representative will return your call.

All co-payments are due at the time of visit. If you do not have your co-payment at time of visit, your appointment will be rescheduled.

NEW CANCELLATION POLICY: Please give at least 24 hours notice for appointment cancellations. Any appointments that are cancelled at the last minute or with less than 24 hours notice will be subject to a $35 Late Cancellation Fee.

We accept cash or checks.  Credit/debit cards are processed through PayPal (a PayPal account is NOT required). Select the Pay Online button to the left.

If desired, our front desk staff can process your credit/debit card payment for you at the time of visit (the cardholder must present card personally).

ATMs are located across the street for your convenience.

We do not participate in HMO plans. Please call the office prior to your visit to confirm participation with your health plan.

Most insurance policies require the patient to pay a small percentage of the office visit cost; this is your Co-Pay.

It is important that you bring your insurance card and co-payment to each visit. Copayments, deductibles and non-covered charges are due at time of visit.
